How can I find out what medals I am authorized?
Form 201A lists your medal, award and training authorizations. To obtain Form 201A which you must mail or fax a copy of your Report of Separation (DD-214) to The War Library. Include your full email address. Very soon after sending your Report of Separation your Form 201A will be emailed to you. You should immediately print and staple your 201A Form to the back of your DD-214.
